The Statewide Evaluation and Planning Services (STEPS) program provides comprehensive evaluation and assists with care plans for individuals requiring long-term care. Evaluations are conducted to determine whether community-based services could appropriately substitute for nursing facility care.


The STEPS evaluation process includes:

  • A comprehensive evaluation
  • A multidisciplinary assessment, including the establishment of a plan of care
  • Financial eligibility screening
  • Providing information and assistance regarding available services which could substitute for nursing facility care

Please note that the assessment is completed in whatever location the client is residing.


Any resident of Baltimore County in need of a long-term care assessment or considering nursing home placement is eligible for a STEPS assessment.


There is no cost to clients seeking an assessment. Baltimore County is reimbursed by the State of Maryland for eligible clients.


To enroll in the STEPS Program, call 410-887-2754, Monday through Friday, from 8:30 a.m. to 4:30 p.m.
