- Benefits Change—Active employees can use this form during the year when they have a change to their benefits status. Complete the form online then print a copy of it. Sign and date the completed form and attach documentation for the requested change.

Davis Vision Plan Updates
CareFirst Davis Vision members have access to:
- Hearing Discounts—Discounts on hearing aids and services through Davis Vision and EPIC Hearing Healthcare, including hearing aid savings of 30 to 60 percent off retail price, and access to a national network of ear physicians and audiologists. To enroll, call 1-844-246-0544 or email hear@epichearing.com.
- Lasik Services—Significant savings through Davis Vision and QualSight, including up to 50 percent savings off the national average price for Traditional LASIK and access to over 1,000 locations nationwide. Speak to a QualSight Care Manager by calling 855-502-2020 or by visiting the Davis Vision home page.
About the Health Insurance Division
The Health Insurance Division is responsible for the administration of the County's medical, prescription, dental, vision, life insurance, flexible spending, voluntary long-term disability and Commuter Choice plans. The Division also coordinates and assists in budgetary management and contract negotiations for health insurance for:
- Baltimore County Public Schools
- Community College of Baltimore County
- The Revenue Authority
- Baltimore County Public Library
These activities ensure the lowest costs and best value for the County. The County's goal is to promote the informed and cost-effective use of the benefit plans it sponsors.
