Voluntary Long-Term Disability Insurance 

Effective January 1, 2020, all benefit-eligible employees will have access to Cigna’s Voluntary Long-Term Disability (LTD) Insurance, which provides income protection for unexpected events through a 100 percent employee-paid premium. Employees who are absent from work for 90 consecutive days for an illness or injury may submit a claim for LTD benefits. You are not required to exhaust your accrued leave before applying.

If the claim is approved, 60 percent of your gross monthly base salary (up to $7,500) will be paid when you become disabled and can’t work for a period of time due to a covered illness or injury. Payments come directly to you or someone you designate, and can be spent any way you like, either for planned expenses, such a groceries, mortgage or utilities, or unplanned expenses like medical bills. Use Cigna's online Disability Income Needs Calculator to help you evaluate your income protection needs​.

Rates

The Cigna LTD 100 percent employee-paid rates are as follows:
  • Monthly rate per $100: $0.632
  • Biweekly (24 pays) rate per $100: $0.316

Below is an example of how to calculate the biweekly rate per pay:

  1. Calculate your monthly salary. (Example: $3,000)
  2. Divide by $100. (Example: $30)
  3. Multiply by .316. (Example: $9.48 per pay)

How to File a Claim

You can file your claim by phone or online.

By Phone

Speak to a representative toll-free by calling 888-36-Cigna (24462).

Online

Fill out an online claim form by following the steps below if you have Cigna health insurance:

  1. Log in to myCigna.com.
  2. Select Coverage.
  3. Select Disability/Leave of Absence.
  4. Select Submit a Disability Claim.

If you do not have Cigna health insurance:

  1. Visit Cigna's Customer Forms page.
  2. Select Disability/Accident/Life/Critical Illness/Hospital Care Forms.
  3. Select Submit a Disability Claim, which will bring you to the Fraud Warning page.
  4. Review and select Continue at the bottom of the page.
  5. A pop-up box will appear that says "Hit the continue button if you have read the above fraud language and wish to continue to file a claim.”
  6. Select Continue.
  7. Select Submit a disability claim online to begin.

How to Enroll

Eligible employees may enroll in Voluntary Long-Term Disability online through Employee Self Service. New employees that would like to enroll may do so within the first 31 days of hire.
