Removal of Licensee
The following documents must be submitted to the board in order to remove a licensee from a license without replacement.
- Affidavit of Resignation or Statement of Withdrawal from the business by current licensee to be removed
- Sales Agreement effecting a stock transfer or documentation regarding the purchase of current licensee interest in the business
- Corporate Minutes effecting corporation changes and the election of officers
- Completed Application and financial Affidavits for remaining licensee(s) with necessary information effecting the change
