Removal of Licensee

The following documents must be submitted to the board in order to remove a licensee from a license without replacement.

  1. Affidavit of Resignation or Statement of Withdrawal from the business by current licensee to be removed
  2. Sales Agreement effecting a stock transfer or documentation regarding the purchase of current licensee interest in the business
  3. Corporate Minutes effecting corporation changes and the election of officers
  4. Completed Application and financial Affidavits for remaining licensee(s) with necessary information effecting the change
