Case Management
Case management is a client-centered service that links clients with medically appropriate levels of health care and support services. A social worker or nurse provides access to a continuity of care, ongoing assessment of the clients’ needs, and personal support systems to insure that clients remain in care. Persons who think they need case management can call us for an intake at 410-887-2437. A brief phone interview begins the process. Proof of being HIV positive, proof of income and proof of residency are required.
The following are some of the typical services which may be provided:
- Medical Care -- Ryan White funding provides medical care for HIV+ persons. We can also help locate a medical provider who specializes in treating HIV.
-- Help obtaining medication or special insurance programs. - Housing Vouchers -- clients have access to vouchers, like Section 8 vouchers, to find housing in the community through a grant from Housing and Urban Development (HUD). There is currently a waiting list for this program.
- Emergency Financial Services -- such as utility cut-offs; funding available to help people remain in the community or prevent eviction
- Mental Health Services -- through a contractual agreement with Johns Hopkins Hospital.
- Legal Services -- through a contractual agreement with HERO.
- Transportation Assistance -- for eligible person to get to medical appointments
- Substance Abuse Evaluation and Referral
- Horizons Support Group (PDF) -- functioning since 1988, this group provides a safe place for clients to share experiences and gain hope in dealing with their disease. This is open to ANY HIV positive member. You do not need to be receiving services in order to participate in the group.
A Client Advisory Board provides consumer input and advocates on behalf of the HIV+/AIDS-infected Baltimore County residents.
