Back to top Current Maryland law requires all providers use the Maryland Department of Health and Mental Hygiene-approved consent forms for all patients being tested for HIV. The AIDS Administration has just launched a new Perinatal HIV consent form, the Perinatal HIV Authorization Form (PDF). It is a streamlined version of the universal counseling and testing informed consent form. The shorter form should reduce the time needed to educate women about HIV and obtain informed consent for testing.
