Disease Reporting in Maryland
The Maryland Confidential Morbidity Report Form (DHMH-1140) (PDF) is available for downloading and reproducing on the Maryland Department of Health and Mental Hygiene’s web site. Disease reporting instructions and listing of notifiable diseases, can be obtained from the Community Health Administration, Epidemiology and Disease Control Program.

HIV Consent Form
Current Maryland law requires all providers use the Maryland Department of Health and Mental Hygiene-approved consent forms for all patients being tested for HIV.
The AIDS Administration has just launched a new Perinatal HIV consent form, the Perinatal HIV Authorization Form (PDF). It is a streamlined version of the universal counseling and testing informed consent form. The shorter form should reduce the time needed to educate women about HIV and obtain informed consent for testing.
