The program provides home-based nursing services for infants and children to age 3 years who are at risk for preventable illness, disability or developmental delays due to medical, social or environmental problems.
Nursing services include a health and developmental assessment, anticipatory guidance, health education and counseling based on the plan of care for the identified problems. These services are provided in collaboration with medical services by the primary provider. Infants are referred from hospital neonatal intensive care units, private medical physicians, social workers and other home health agencies. Coordination with other disciplines and organizations' resources is an important component of the total services.
Request for services are made using an Infant Identification form or calling 410-887-2705. Referrals are triaged to the high risk infant nursing team who serves the entire Baltimore County community.
Home visits are provided between 8:30 a.m - 4:30 p.m., Monday through Friday.
