The goal of Administrative Care Coordination Unit (ACCU) is to improve the utilization of health care services among various vulnerable populations in the county. The unit Provides education and short term case management to clients accessing care in Health Choice.
The unit assists HealthChoice MCOs in providing care for HealthChoice enrollees by providing education, advocacy and follow-up care services for non-compliant clients. The staff also provides education to community groups, schools and other health care providers. It also provides limited case management required by the Healthy Kids (EPSDT) Program to infants/children up to 21 years of age to ensure access to preventive health services.
The ACCU will work collaboratively with the health care providers/MCOs to bring such non-compliant clients back into care. This collaborative effort will continue until the client is back into compliance.
- Children birth to 2 years who have missed two consecutive Healthy Kids appointments.
- Children under 21 years including children with special health care needs; who have missed two consecutive appointments for follow-up treatment.
- Pregnant women who have missed two consecutive prenatal appointments.
- Other members of special populations as defined in HealthChoice who have missed three consecutive appointments. These populations include:
- individuals with physical disability
- individuals with a developmental disability
- individuals who are homeless
- individuals with HIV/AIDS
- individuals with a need for substance abuse treatment
The public health nurses serve consumers in the entire Baltimore County area.
Services are provided Monday through Friday 8:30 a.m. - 4:30 p.m.
