The Baltimore County Department of Health, Bureau of Long Term Care has identified medication mismanagement in the elderly as a major health concern. This mismanagement is detrimental to the health and independence of our senior citizens.

Medication mismanagement can lead to unnecessary use of the emergency room, increased hospitalizations, and premature nursing home placement. A significant obstacle to remaining in the community is the inability to self-medicate, or the development of health problems due to medication mismanagement.

Services

Project M.E.D. is an enhanced medication program which includes home-based client education and instruction in the use of medication. More specifically, an in-home nursing assessment is completed, with the development of an individualized medication management plan. Other services provided are the education of clients and/or caregivers concerning relevant medication issues, and exploration and possible purchase of appropriate compliance products.

Eligibility

This program is available to Baltimore County residents who are at least 60 years of age. Clients must have an attending physician and be interested in improving their medication compliance.
