The PARTNERSH.I.P. provides enrollment and links to community physician or other health services. A plan of care is developed, resources and education are provided, and the resident becomes a participant in his/her own health care plan. The program is able to provide access to quality, affordable health care for approximately 600 - 700 members.
Case management and coordination of health care services for uninsured adults with incomes below 200 percent of the federal poverty level.
Baltimore County resident between ages of 19 - 64 with income below 200 percent of the federal poverty level.
