The Medical Assistance Transportation program is funded by the State of Maryland and enables eligible individuals to access needed medical care by providing transportation services.
Medical Assistance Transportation coordinates transportation services for Medical Assistance recipients who reside in Baltimore County and who are in need of a transportation to and from medically necessary services. The program provides "last resort" transportation to ambulatory, wheelchair, and stretcher-bound recipients who have no other means available to get to and from medical appointments.
This service is available to Baltimore County residents with eligible, non-restrictive Medical Assistance (Medicaid) to medical services that are covered by Medical Assistance and for which there is no other means of transportation. A Physician Certification for Medical Transportation is required for all clients.

Physician Certification for Medical Transportation (PDF) - Download, complete, and have this form signed by the recipient's doctor. A doctor must certify that the patient is unable to use public transportation due to a specific medical condition. The Maryland State Department of Health and Mental Hygiene requires medical certification for all recipients. Once the form is completed and signed it should be faxed or mailed to the Baltimore County Department of Health.
Dialysis Renewal Fax Request (PDF) - An outline of procedures for renewing existing dialysis requests. This renewal form should be downloaded, completed then faxed or mailed to Baltimore County Medical Assistance Transportation by the 3rd week of the month to insure that dialysis transports continue uninterrupted.
Pending Medical Assistance Agreement (PDF) - This form should be downloaded, completed, and signed by the Nursing Home Facility Administrator. The Maryland State Department of Health and Mental Hygiene encourages local transportation programs to enter into agreements with nursing homes in their county in order to provide necessary transportation services for recipients whose Medical Assistance (Medicaid) is in pending status.

Any Baltimore County residence. Transportation services are provided to recipients who reside in a private home, group home, nursing home or other type of residence within Baltimore County. Recipients will be picked up from their place of residence and transported to their medical appointment and returned back to their residence.
