The Home Health Services Program enables acutely and chronically ill residents to be monitored for changes in their condition to prevent costly nursing home or hospital admissions. Home based client education and close monitoring and communication wiht personal physicians are the focus for the services provided. Home Health Services works with all available resources and is one step in a continuum of care within the Bureau of Long Term Care. The program is licensed by the State of Maryland as a Medicare certified Home Health agency.
Home Health Services (PDF) provides intermittent skilled nursing, physical therapy, occupational therapy, speech therapy, medical social service, and home health aide to homebound Baltimore County residents under the direction of their physician.
Clients must be homebound, have a need for skilled services and be under the care of a physician to be eligible for Home Health Services.
