Hospital Support Services provides discharge planning services to Baltimore County residents hospitalized at Franklin Square Hospital Center, Northwest Hospital Center, Greater Baltimore Medical Center, St. Joseph Medical Center, or Johns Hopkins Bayview Medical Center.
Public health nurses assist clients in conjunction with hospital staff and physicians in coordinating a post-hospitalization plan of care to provide a smooth transition from hospital to home with the implementation of needed services and resources such as home health nursing, physical therapy, etc.
This service is provided to Baltimore County residents hospitalized at Franklin Square Hospital Center, Northwest Hospital Center, Greater Baltimore Medical Center, St. Joseph Medical Center or Johns Hopkins Bayview Medical Center.
There is no charge for this service.
