| |  | The STEPS Program provides comprehensive evaluation and assists with care plans for individuals requiring long term care. Evaluations are conducted to determine whether community based services could appropriately substitute for nursing facility care. The STEPS evaluation process includes:
(The assessment is completed in whatever location the client is residing.) - a comprehensive evaluation;
- a multidisciplinary assessment, including the establishment of a plan of care;
- financial eligibility screening; and
- providing information and assistance regarding available services which could appropriately substitute for nursing facility care.
| | Any resident of Baltimore County in need of a
long term care assessment or considering nursing home placement is eligible for a STEPS assessment. | NO cost to clients seeking an assessment. Baltimore County is reimbursed by the State of Maryland for STEPS eligible clients. |
