| |  | This is a federally mandated program designed to identify, assess and determine an appropriate plan of care for those individuals with a diagnosis of mental illness and/or mental retardation seeking nursing home placement. Clients already in nursing homes are subject to resident review if they have a diagnosis of mental illness and/or mental retardation and experience a significant change that would impact their need for nursing home placement and/or specialized services. - An assessment is completed by a nurse and/or social worker.
- A psychiatric consultation is completed for those individuals with mental illness.
- A psychological evaluation is completed for those individuals with mental retardation.
- Final determination to address the need for nursing home and/or specialized services is made
by the Mental Hygiene Administration and/or the Developmental Disability Administration.
