The Case Management Program, staffed by registered nurses and licensed social workers, provides the following services which are designed to assist at-risk clients in need of long-term care services to remain in the community: - assessments;
- care plan development;
- coordination of services; and
- monitoring of those services to enable clients to remain in the community safely.
Any Baltimore County resident needing assistance to coordinate services enabling them to remain in a non-institutional setting may be referred for case management services. > | A comprehensive evaluation is completed which includes medical/nursing, functional, psychosocial, financial and environmental components | > | The multi-disciplinary team assists the patient, family, personal physicians and others by developing an appropriate plan of care to help the client remain in the least restrictive environment. | > | The coordination and implementation of services and follow-up is included in the case management program.* | There is NO cost for the assessment and case management services. *Coordinated services are on a fee-for-service basis or publicly funded depending upon eligibility and availability. |
