Case Management
Case management is a client-centered service that links clients with medically appropriate levels of health care and support services. A social worker or nurse provides access to a continuity of care, ongoing assessment of the clients’ needs, and personal support systems to insure that clients remain in care.
Persons who think they need case management can call us for an intake at 410-887-2437. A brief phone interview begins the process. Proof of being HIV positive, proof of income and proof of residency are required.
Health and Social Services
Through case management, clients may be able to receive some of the following services:
Dental Services - Dental care given by University of Maryland Dental Clinic at Liberty Family Resource Center
Emergency Financial Services - Help to pay utility bills and for other emergencies
Housing Assistance - Emergency help for evictions and a limited number of housing vouchers
Legal Services - Legal services by appointment only
Medical Care - Care given by Johns Hopkins Hospital working with Baltimore County Department of Health or referral to other providers
Medications - Emergency help with payment for medication
Outreach Services - Counseling and referral services for those who have dropped out of care or for those not yet getting medical care and other needed services
Mental Health Services - Counseling at our clinic sites or referral to other providers
Substance Abuse Treatment - Referral to treatment centers that offer special funding for those with HIV
Horizons Support Group - Friendship and support given through this group (Transportation is provided if needed and refreshments are served at no cost)
Transportation Services - Transportation provided for medical appointments
Client Advisory Board (CAB)
A group of organized volunteers who are dedicated to educating the Baltimore County community on HIV prevention and related issues. The mission of the CAB is to advocate on behalf of the HIV+/AIDS-infected Baltimore County residents who utilize the Baltimore County Department of Health to access a comprehensive array of high quality, culturally-sensitive, supportive social services that will promote “wellness” and assure client accessibility to informational/educational materials and resources that will prevent disease and disability.
