To request a copy of a fire incident report, print this Fire Incident Report form and carefully follow the printed instructions, including the instructions for returning it by mail. 

If you need a patient care report, proceed to the Patient Care Report list below and, and select the link that applies to you. 

Patient Care Report

 Select which best applies to you: 

I am an attorney 

I am the patient 

I am a parent of a patient who is a minor child  

I am a court-appointed guardian or custodian of the patient

I am a personal  representative of a deceased patient

I am authorized to consent to health care for the patient 

Please e-mail the Baltimore County Fire Department Records Division or call  us at 410-887-4599, weekdays from 8 a.m. to 4 p.m., if you are:

• someone with a different relationship to the patient than those listed above
• with a law enforcement agency, regulatory agency or other private or governmental entity
• none of the above

Maryland law requires that we respond to your request within 20 working days. The Fire Department makes every effort to respond to requests as quickly as possible.
