General Information

The Baltimore County Fire Department's policies and procedures for the release of Patient Care Reports are designed to comply with state Public General Law, while not placing unnecessary burdens upon those seeking copies of records.  (Maryland Code, Health-General, Title 4. Statistics And Records, Subtitle 3, Confidentiality Of Medical Records)

Help us provide you with the report you need in a timely fashion by sending all of the required items and payment. Now, please choose the description that describes you from the list below in order to access the appropriate patient care report request form.

I am an attorney representing:

• A patient
• A person authorized to consent to health care for an adult or minor child
• A parent of a minor child
• A court-appointed guardian, custodian, or representative of a minor child
• A duly appointed personal representative of a deceased adult or minor
