Request for Patient Care Report by an Attorney
General Information
The Baltimore County Fire Department's policies and procedures for the release of Patient Care Reports comply with state Public General Law. At the same time, the Department seeks to make it as easy as possible to obtain records. (Maryland Code, Health-General, Title 4. Statistics And Records, Subtitle 3, Confidentiality Of Medical Records)
You can help us provide the report you need in a timely fashion by sending all of the required items and payment.
From the list below, choose the description that fits your role to access the appropriate patient care report request form.
I am an attorney representing:
- A patient
- A person authorized to consent to health care for an adult or minor child
- A parent of a minor child
- A court-appointed guardian, custodian, or representative of a minor child
- A duly appointed personal representative of a deceased adult or minor
