Advanced Life Support (ALS) Recertification
Please read entire document for complete instructions on filling out your ALS recertification application.
All ALS Providers whose card expires March/April, will be receiving their Recertification Paperwork in the mail very soon. If you do not receive it by Dec 15, download an application from the National Registry's web site: www.nremt.org, fill it out, sign it and send it to the FRA.
Please complete the application and send it to the FRA as soon as you get it. Please do not wait until you finish ALS Con Ed. We will verify your attendance prior to mailing the completed form to the National Registry.
Follow the directions on the National Registry application. Here is some additional information to help you accomplish this.

Registry Number: Be sure your registry number is written in. (This can be found on your card OR on the return address label included in your application.)

Paramedic Refresher Training (48 hours)
EMT Intermediate/99 Refresher Training (36 hours):
Include a copy of your MFRI certificate. If you do not have it yet, we will include a certificate for you as long as you attended classes sponsored at the FRA.
If you attended an ALS Recertification class outside of Baltimore County, you will need to submit documentation of refresher completion.
CPR Certification: Career: FRA Staff will sign
Volunteer: Need a copy of your CPR card.

ACLS Certification: Send a copy of your ACLS card OR ACLS Instructor card. If you took Actronics at the FRA, you should have printed out your certificate before leaving. The FRA should have a copy of it. If you took ACLS outside of the FRA/Actronics, you will need to send a copy of your ACLS card.
Additional EMS Related Continuing Education:
YOU MUST FILL THIS IN. You should have all the dates you attended training. If not, your station log or EMS Supervisor should have this information. Remember, ANY training counts, to include in-station training such as: SCBA, fire skills, computer training, physician led classes, etc.
EMT-Ps need 24 hours
EMT-Is need 36 hours

Verification of Skill Competency: The FRA will fill out.
Sign the form (on the bottom of Page 2).
Page 3 and Page 4: are your option to fill out.
Make a copy of the application for your records, (we strongly recommend) and send the application, documentation copies, and return address label to the Fire Rescue Academy.
All completed applications must reach the FRA by February 1. Failure to do so could result in your cost of $20/$15 for re-registration fees.
