Homeless Management Information System (HMIS)
The implementation of the Homeless Management Information System (HMIS) is a Congressional mandate for all jurisdictions that receive federal funding for homeless and homeless prevention services. The Baltimore County HMIS is a cooperative community case management tool that allows governmental agencies and non-profit organizations to better serve their homeless clientele. Baltimore County uses Bowman System's Service Point to deliver its' HMIS services. - Baltimore County Universal Intake Form (PDF) - HMIS intake form to be completed for all adult clients. All questions in bold black are required to have an answer.
- Baltimore County Universal Intake Form / Children (PDF) - HMIS intake form to be completed for all child clients. All questions in bold black are required to have an answer.
- Consent Form (DOC) - must be signed by all adult clients. For HMIS users, be sure to fill out your agency name in the appropriate fields.
- HMIS Agencies (XLS) - Reference guide used to find services provided by agencies in Baltimore County using HMIS.
- HMIS Privacy Notice (PDF) - Must be posted at all locations where HMIS intake is performed.
- HMIS Privacy Policy (PDF) - Clients have the right to receive a copy of this policy at any time.
- Service Point Workflow (PDF) - Manual used to guide users on the correct data entry procedure. Agencies that do not provide shelter or housing use this manual.
