Contacts/Links To Contact your Benefit Plans

In many cases you will need to contact your benefit plan directly for answers to your questions. The telephone numbers for their customer service departments are included on the Plan ID Cards.

Examples of when you should contact your health plan directly are:

  • When you want to make a change in the Primary Care Physician (PCP) you or a dependent has chosen
  • When you need any other information about the providers in their networks
  • When you want information regarding the coverage for a specific health care or dental care service
  • When you don't understand how or whether a claim was paid correctly on your behalf
  • When you suspect fraudulent billing for services you did not receive

The Plan web sites contain valuable information about your benefits, their providers and even resources to assist you with making healthy lifestyle choices. It's a good idea to "bookmark" your plan's site so that you can stay informed about upcoming events and services they provide.
