The Department of Aging provides free, gap-filling casework services to homebound seniors to help them remain in their own homes and thus prevent premature or inappropriate institutionalization. Community Outreach Specialists provide home visits, crisis intervention and emergency assistance, counseling, advocacy, assistance completing benefits applications and housing-related services to homebound seniors.
They develop care plans to meet the clients' needs, using available services and funding sources. Home-delivered meals, in-home aide services, rent and fuel subsidies, and assistance with home repairs are examples of some of the services utilized. Through the Senior Care program, which includes federal and state funds, as well as the Fund for Needy Seniors, Community Outreach Specialists are able to purchase needed medication, hearing aids and other health-related items, and additional services to help clients remain in their homes. Clients are monitored and reassessed regularly.
Community Outreach Specialists also offer assistance to seniors, families and caregivers to determine eligibility for various programs and services at all 19 senior centers.They help access such benefits as Medical Assistance, Pharmacy Assistance, food stamps, subsidized housing, home repair deferred loans, the Maryland Energy Assistance Program and tax credits for homeowners and renters.
