Chronic Disease Self Management Program and Diabetes Self Management Program
Take Charge of Your Health: Living Well
Through the Maryland Department of Aging, National Council on Aging, and sanofi-aventis, Baltimore County Department of Aging (BCDA) is offering an evidenced-based health promotion program on either chronic disease or diabetes self management. These programs are a six-week workshop developed by Stanford University.
Self Management workshops help people over 18 years of age, diagnosed or self diagnosed with chronic conditions or diabetes. Workshops help to teach skills to manage their symptoms and improve quality of life. Topics for the CDSMP workshop include: medications, nutrition, exercise, frustration, healthcare communication, future planning and relaxation methods.
DSMP special topics include: techniques to deal with the symptoms of diabetes, fatigue, pain hyper/hypoglycemia, stress and emotional problems such as depression, anger, fear and frustration.
Participants will make weekly action plans, share experiences and help each other solve problems they encounter in creating and implementing their self-management program.
